Family Centered Healthcare has established the following policies to ensure transparency in our administrative practices and our expectations.
LATE ARRIVAL - If you arrive more that 15 minutes past your appointment time you may be asked to reschedule.
Cancelation - Life happens! All we ask is for a heads up if you are not going to make it and need to reschedule.
Controlled Substance Medication - You are REQUIRED to be seen EVERY 3 months to refill a controlled substance.
Medication Refill - Contact your pharmacy to submit a request, send a message through Athena or give the office a call.
